COVID-19 Vaccine Clinic at The Madeleine, December 1

The Madeleine Parish is pleased to announce a second COVID Vaccine Clinic in partnership with Providence Health on Wednesday, December 1, from 2:30–6:00 pm in The Madeleine Parish Hall.

At the clinic, Providence will offer Pfizer, Moderna, and Johnson & Johnson vaccines, and will be able to provide adult and pediatric first and second doses, along with booster doses for those eligible (click here for booster eligibility from OHA). 

Please complete this Sign-Up Genius so that Providence has a better understanding of how many people will attend. The signup is in 15-minute increments. The signup will ask which vaccine dose and which vaccine manufacturer you would like to receive. Please complete the signup ONCE for each person who will be attending the clinic.

If you have received any vaccine doses, be sure to bring your original vaccine card to the clinic, so it can be updated with additional notations.
